This program is brand new and in its first year, located in our satellite office in the Riverfront Center building in Amsterdam.

Within Healthy Families of Montgomery and Schoharie Counties, our free and voluntary home-based services and resources are designed to assist expectant families with creating a healthy home. Healthy Families of Montgomery and Schoharie Counties is an evidence-based program, and the research shows that home-visiting can positively impact the lives of children and their families — families have healthier babies, demonstrate better knowledge of parenting and child development, create positive family bonds, develop connections to community resources, and have children who perform better in school.

Families who participate in Healthy Families of Montgomery and Schoharie Counties are offered in-home support services until the child enters Kindergarten, Head Start or turns 5 years old. A primary goal of this program is to identify strengths and challenges prenatally or at birth, and to provide appropriate information, care, resources and referrals to help babies get off to a healthy start.
A las familias que participan en Healthy Families de los Condados de Montgomery y Schoharie se les ofrecen servicios de apoyo en el hogar hasta que el niño ingresa al Jardín Infantil, al Head Start o cumple 5 años de edad. Uno de los principales objetivos de este programa es identificar fortalezas y desafíos antes del nacimiento o al nacer, y brindar información, atención, recursos y remisiones adecuadas para ayudar a los bebés a iniciar su vida de forma saludable.

Must be pregnant or have a partner who is pregnant, or a child aged 3 months and younger.

Making A Referral | Hacer una Remisión
Anyone can make a referral to HFMSC. You can even refer yourself! We strive to talk with all expectant parents and parents of newborns in Montgomery and Schoharie Counties. Look for our surveys (referral forms) throughout the community, (i.e., prenatal clinics, WIC, doctor’s offices, etc.)
Referrals can be made by completing a program survey and sending by fax to (585) 684-5816, by email to sbellassai@ican.family, or by calling (315) 801-7357. Self-referrals are accepted and encouraged!

Access To Resources
We connect individuals and families with critical community resources, such as access to physical and mental health providers, employment opportunities, housing options, peer support groups, and Wraparound services. We understand that access to some of these services can be difficult to attain during certain life challenges, and we are here to help individuals and families overcome this obstacle.
