Kids Otsego Service Coordinator

Kids Oneida, Inc., a Community-based WRAPAROUND provider of Social and Mental Health services with emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill multiple Kids Otsego Service Coordinator positions in the Kids Otsego program.  

The Kids Otsego Service Coordinator oversees all activities related to the development and implementation of individualized service plans in a preventive context and promotes the philosophy and missions of the company by performing the following duties:

Duties & Responsibilities:

  • Conducts outreach to engage clients and assist families with an understanding of agency philosophy and policies.
  • Responsible to travel within the county and meet with clients within homes and schools.
  • Links clients and their families to appropriate resources and services in a timely and coordinated manner.
  • Assists the client and family with the development, implementation and monitoring of their individualized service plan of schedules meetings to review, revise and discuss such plans as required.
  • Schedules monthly team meetings with the family, service providers and any other individuals identified as a team member.
  • Acts as a liaison between client and family, agency personnel, DSS staff and other service providers as necessary and appropriate.
  • Provides advocacy for the development of services and resources that do not currently exist within the community but are deemed necessary.
  • Identifies and utilizes both formal and informal community supports and networks.
  • Completes all required documentation in a comprehensive and timely manner as required by DSS and KO regulations including mandatory face to face contacts.
  • Attends agency in-services, workshops and seminars as required.
  • Attends family court as necessary.

Education/Experience:

  • Bachelor's degree (B. A.) in human services or mental health field from four-year college or university; with at least two years related experience and/or training; or equivalent combination of education and experience.

Parent Partner

Kids Oneida, Inc., a Community-based Wraparound provider of social and mental health services with emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill a part time Parent Partner position.

The Parent Partner works as part of multidisciplinary team and provides peer-to-peer support and interventions to foster parent/caregiver engagement and empowerment within a variety of service delivery systems.  Brings life experiences and skills to the position to enhance the team effort to deliver assistance to the family as they explore the goals they would like to achieve, by performing the following duties.

Duties & Responsibilities:

  • Draws on his/her own experience of raising a child with emotional or behavioral issues as they work with the parents/caregivers and the team to discover the family's values and priorities.
  • Deliver interventions and activities that meet needs, build on strengths, and achieve outcomes.
  • Responsible for assisting parents/caregivers with engaging in the enrollment process by utilizing knowledge and experiences encountering various service systems (i.e. school, mental health, juvenile justice, etc.). Help parents/caregivers assess their available resources and service needs.
  • Coach parents/caregivers on self-advocacy skills (i.e. delivering messages clearly and with the desired impact, self-expression, active participation in service plan development, etc.).

Education/Experience:

  • High school diploma or general education degree (GED); or two to four years related experience and/or training; or equivalent combination of education and experience.
  • Parent or caregiver who are raising or have raised a child with mental health concerns and are personally familiar with the associated challenges.
  • Credentialed Family Peer Advocate, preferred.  Required to complete the Family Peer Advocate credentialing process within 12 months of hire.

Evelyn’s House Service Coordinator

Kids Oneida, Inc., a Community-based WRAPAROUND provider of Social and Mental Health services with an emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill an Evelyn’s House Service Coordinator position.  

The Evelyn’s House Service Coordinator Oversees activities related to the development and implementation of individualized plans of care and adheres to and promotes the philosophy and missions of the program by performing the following duties and responsibilities:

  • Provide individualized direct services for adolescent mothers and their children.
  • Link clients to appropriate resources and services in a timely and coordinated manner.
  • Develop individualized service plans and meet regularly with each resident.
  • Maintain a comprehensive case record for each participant, documenting needs, challenges, and achievements.
  • Assist residents to achieve program goals by providing guidance and encouragement.
  • Make referrals and advocate to ensure that all residents receive the necessary services.
  • Provide transportation for participants to attend appointments and educational programs.
  • Assist the program director with coordinating in-service trainings offered by community partners.
  • Assist the program director in coordinating group meetings and educational sessions.
  • Assist program director with the completion of federal and state reports.
  • Act as a liaison for the program by attending community meetings with a focus on improving services and resources for residents.

Education/Experience:

  • Bachelor's degree (B. A.) in human services from four-year college or university; or two to four years related experience and/or training; or equivalent combination of education and experience. Documented experience working with teen parents, young mothers, and families with children required. Knowledge of local systems of care preferred.

Community Integration Counselor- Part Time Per Diem

Kids Oneida, Inc., a Community-based WRAPAROUND provider of Social and Mental Health services with emphasis on treating the client in a family context, is currently seeking innovative and experienced applicants to fill part time per diem Community Integration Counselor positions in the Nursing Home Transition and Diversion (NHTD)/ Traumatic Brain Injury (TBI) waiver program.

Community Integration Counselors provide individualized services designed to assist TBI and NHTD waiver participants; to help them manage and overcoming the difficulties confronted by a change in physical and cognitive abilities while living in the community. Community Integration Counseling provides a participant with a collaborative counseling service to learn to cope with altered abilities and skills while adhering to and promoting the philosophy and mission of the company by performing the following duties:

  • Supports and encourages individualized ability to problem solve, be in control of life situations and to be as independent as possible.
  • Meets with waiver participants on a regular basis either in the individual’s residence, the community or an office setting.
  • Provides collaboration with participant, natural support and other waiver providers in support for integration efforts.
  • Documents individual’s progress toward acquiring community integration.
  • Assists in building community based support.
  • Utilizes evidence based therapeutic interventions and rapport-building practices, including motivational interviewing techniques, to enhance participants and family insight regarding diagnostic and usage practices.
  • Provides individualized strength based care and assessment.
  • Works to develop innovative and therapeutic treatment interventions including providing guidance to other waiver providers. Maintaining open communication with Service Coordinator, natural support and others as necessary.
  • Meets quality expectations of accuracy and completeness while being responsive to feedback.
  • Meets timeliness expectations of completing work as scheduled, demonstrating self-paced performance and adjusting priorities as needed.
  • Provides all care while adhering to social work ethics and values.
  • Attend team meetings for all participants

Education/Experience:

  • Educational Experience: Licensed Psychiatrist, Licensed Psychologist, Master of Social Work, Master or Doctorate of Psychology, Certified Rehabilitation Counselor, Master of Counseling Psychology, Licensed Mental Health Practitioner or Certified Special Education Teacher.
  • Experience Requirements: At least one year of experience providing adjustment related counseling to individuals with traumatic brain injury, those with disabilities and/or seniors
  • Must be an Individual who is highly motivated, independent, and professional, organized, and has strong writing and communication skills.

Receptionist/Administrative Assistant

The Receptionist/ Administrative Assistant relieves management and direct line staff of clerical work and minor administrative and business functions.  Maintains a positive working presentation and relationship with the public/community that promotes a positive agency image by performing the following duties:

  • Ensures effective communication across multiple suites to help facilitate day to day business matters.
  • Responds promptly to incoming guests as appropriate.
  • Answers telephone and provides information/assistance or routes call to appropriate staff member.
  • Takes telephone or visitor messages and delivers to appropriate individual.
  • Operates standard office equipment efficiently to include computer, printer, photocopy and fax machine.
  • Assists with program data entry as requested by management.
  • Orders and monitors office supplies.
  • Maintains master forms designed to enhance efficient office operations by applying knowledge of software applications.
  • Maintains an updated and accurate calendar as well as schedules appointments and office meetings as requested.
  • Opens, date stamps and sorts and distributes mail.
  • Prepares outgoing mail using postage meter.
  • Assists program staff with reasonable requested tasks.

Education/Experience:

·       Associate's degree (A. A.) or equivalent from two-year college or technical school; or one to two years related experience and/or training; or equivalent combination of education and experience.
